This form should be filled out by your team representative. Select either United States Dragon Boat Open Championship (USDBOC), Regular Division, or a specific Invitational.
Note: registration for Invitational races does not guarantee participation; entries are granted at our discretion.

Schedule team practice at Meadow Lake
Once registered and paid the registration fee, contact tcdbfnewyork@cs.com to schedule a time slot for your team to practice at Meadow Lake.

This form will record who will be on your competing team. If there are any changes to your team roster, please contact tcdbfnewyork@cs.com.
Sign waivers
To participate in the race, every team member must sign the waiver of Liability Release, Indemnification and Hold Harmless Notice.
Note: Online registration and online team roster forms are strongly preferred; however, in the case that you aren't able to access them, please download and mail the following forms along with the waivers and registration fee.
